Digital copy of student’s most recent vaccination/immunisation record

Recent passport-sized photos (191 pixels * 251 pixels) of student and parent/guardian

If students are under the care of an appointed guardian, parents must provide a copy of the guardian’s most recent passport-sized photograph, NRIC (Malaysian), and letter of guardianship appointment
